These Terms of Service ("Terms") govern your use of the services provided by Become Whole, LLC ("Practice," "we," "us," or "our") through our website located at TherapyWJazz.Org ("Website"). By accessing or using our Website, you agree to be bound by these Terms. If you do not agree, please do not use our Website.

Emergency Notice

This Website and its contact forms are not monitored 24 hours a day, 7 days a week, and are not appropriate for emergencies. If you or someone you know is experiencing a mental health crisis or is in immediate danger, please call 988 (Suicide and Crisis Lifeline), call 911, or go to your nearest emergency room immediately. Do not submit a contact form or email in an emergency.

1. Use of Services

1.1. Eligibility: Our services are available to adults aged 18 and older and to adolescents with parental or legal guardian consent. By using our services on behalf of a minor, you represent and warrant that you are the minor's parent or legal guardian and that you consent to these Terms on their behalf.

1.2. Geographic Limitation: Become Whole, LLC provides telehealth services to clients located in Virginia and any other states in which our clinicians are licensed to practice. By scheduling services, you confirm that you are physically located in a state where we are authorized to provide services at the time of each session.

1.3. Account Information: If you create an account or client profile through our practice management system, you agree to provide accurate, current, and complete information and to keep that information updated.

2. Telehealth Services

2.1. Nature of Services: Become Whole, LLC provides telehealth mental health therapy and clinical supervision services exclusively through HIPAA-compliant video technology. We do not provide in-person services at this time.

2.2. Not a Crisis Service: Our telehealth services are not designed for crisis intervention or emergency situations. If you are experiencing a psychiatric emergency, please contact emergency services as described in the Emergency Notice above.

2.3. Technology Requirements: You are responsible for ensuring that you have the necessary technology, internet connection, and private environment to participate in telehealth sessions. We are not responsible for service interruptions caused by your technology or internet connection.

2.4. Clinician Qualifications: Become Whole, LLC is operated by Jazzmin Maddox, LPC-S, a Licensed Professional Counselor and Approved Clinical Supervisor licensed in Virginia. Any prelicensed resident clinicians providing services through the practice operate under her clinical supervision and are appropriately credentialed under Virginia law.

2.5. Supervision Services: Become Whole, LLC offers clinical supervision to prelicensed counseling residents. Supervision services are governed by a separate supervision agreement, which must be signed prior to the commencement of supervision.

2.6. No Guarantee of Outcomes: Therapy and supervision services are professional services, and we do not guarantee any specific outcome, result, or improvement. Your experience will depend on many factors including your active participation and engagement in the therapeutic process.

2.7. Confidentiality: We are committed to protecting the confidentiality of your health information in accordance with HIPAA and Virginia law. Please review our Notice of Privacy Practices, available in your client portal through SimplePractice, for complete details. You acknowledge that no method of electronic transmission is completely secure and that telehealth carries certain inherent technology risks.

3. Fees and Payment

3.1. Fee Schedule: Fees for therapy and supervision services are disclosed at intake and in your Good Faith Estimate, which you will receive prior to or at the time of your first session as required by the No Surprises Act. Fees are subject to change with advance written notice.

3.2. Insurance: Become Whole, LLC accepts certain insurance plans. It is your responsibility to verify your benefits prior to your first session. You are responsible for any portion of fees not covered by your insurance, including copays, deductibles, and coinsurance.

3.3. Self-Pay: If you are not using insurance, you agree to pay the applicable self-pay rate at the time of service.

3.4. Payment Authorization: By providing a payment method, you authorize Become Whole, LLC to charge applicable fees for services rendered, including late cancellation and no-show fees where applicable.

3.5. Cancellation and No-Show Policy: We require a minimum of 24 hours notice to cancel or reschedule an appointment. Sessions cancelled with less than 24 hours notice or not attended without notice may be subject to a late cancellation or no-show fee. This fee is not billable to insurance and is your personal financial responsibility. The specific fee amount will be disclosed in your intake documents.

3.6. Good Faith Estimates: Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges prior to your first session if you are uninsured or self-pay. Contact us at jazzmin@becomimgwholellc.com to request a Good Faith Estimate at any time.

4. Client Responsibilities

4.1. Compliance with Laws: You agree to use our services in compliance with all applicable federal, state, and local laws and regulations.

4.2. Appropriate Use: You agree not to use our Website or services for any unlawful, harmful, or abusive purpose. You agree not to attempt to gain unauthorized access to any portion of our Website or systems.

4.3. Honest Engagement: You agree to provide accurate information about yourself, your location, and your needs when using our services. Providing false information may affect the safety and quality of your care.

4.4. Location Verification: You agree to confirm your physical location at the beginning of each telehealth session. Receiving services while physically located in a state where we are not licensed may affect our ability to continue providing services to you.

5. Intellectual Property

5.1. Ownership: All content on our Website, including text, graphics, logos, images, and brand materials, is owned by Become Whole, LLC or its licensors and is protected by applicable intellectual property laws.

5.2. Limited License: We grant you a limited, non-exclusive, non-transferable license to access and use our Website for your personal, non-commercial use only. You may not reproduce, distribute, modify, or create derivative works of any content without our express written permission.

6. Disclaimer of Warranties

Our Website and services are provided on an "as is" and "as available" basis. To the fullest extent permitted by law, Become Whole, LLC disclaims all warranties of any kind, express or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that our Website will be uninterrupted, error-free, or free of viruses or other harmful components.

7. Limitation of Liability

To the fullest extent permitted by applicable law, Become Whole, LLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of our Website or services, including but not limited to damages for loss of profits, data, or goodwill, even if we have been advised of the possibility of such damages. Nothing in these Terms limits our liability for gross negligence, willful misconduct, or any liability that cannot be excluded by applicable law.

8. Domain Transition Notice

Become Whole, LLC is currently operating at therapywjazz.org. Our permanent domain, becomewholellc.com, will become active in August 2026. At that time, these Terms will be updated to reflect the new domain. Your rights and the practice's obligations under these Terms remain the same regardless of which domain you use to access our Website.

9. Modification of Terms

We reserve the right to modify these Terms at any time. Changes will be posted to this page with an updated effective date. Your continued use of our Website or services after changes are posted constitutes your acceptance of the updated Terms. We encourage you to review these Terms periodically.

10. Governing Law

These Terms and any disputes arising from them shall be governed by and construed in accordance with the laws of the Commonwealth of Virginia, without regard to its conflict of law principles. Any legal action arising under these Terms shall be brought in the appropriate courts of the Commonwealth of Virginia.

11. Severability

If any provision of these Terms is found to be unenforceable or invalid under applicable law, that provision will be modified to the minimum extent necessary to make it enforceable, and the remaining provisions will remain in full force and effect.
